Tridosha Chikitsa — Treating the Three Doshas
Tridosha Chikitsa is the principle that all Ayurvedic treatment — whether herbal, dietary, lifestyle, or procedural — must ultimately address the dosha imbalance at the root of every disease manifestation.
Ayurvedic therapeutics is organized around the three doshas: every disease is ultimately a manifestation of excess, deficiency, or derangement of Vata, Pitta, Kapha, or their combinations. Treatment therefore aims at three possibilities: reducing excess doshas (Shamana or Shodhana), building deficient doshas (Brimhana and Rasayana), or regulating the qualities of deranged doshas (Balancing through qualities). The elegance of this system is that the same symptoms can have different treatments depending on the dosha involved — a skin rash from Pitta excess requires cooling treatment; the same rash from Vata excess requires warming and moistening treatment. This differential diagnosis based on dosha type produces dramatically different treatment protocols for clinically similar presentations, anticipating modern precision medicine's recognition that the same disease presentation can have different underlying mechanisms requiring different treatments.
The physician who knows the doshas knows the cause of all disease and the direction of all treatment.
Precision medicine's recognition that patients with the same diagnosis (e.g., type 2 diabetes, depression, hypertension) respond differently to the same treatment based on individual differences in genetics, microbiome, and metabolic profile validates the Ayurvedic insistence on dosha-based individualization of treatment.
